LOCAL RESIDENTIAL CLAIMS

What you need to know about filing a claim.
We are sorry that you have found it necessary to report a claim. We appreciate your
cooperation in filling out the form. Upon receipt of the form, a file will be established and
assigned to a claims adjuster. You should receive a letter within 7 days to acknowledge
the receipt of the claim form. United Van Lines / Horizon Moving Systems attempts to settle
all claims in an equitable and timely manner.
General Instructions
A. Please retain all damaged articles, including shipping cartons. These items must be available for inspection.
B. The time limit for filing a claim is 3 months from the date of delivery or conversion to permanent storage, if applicable. The 3-month claim filing period does not apply to government, military, and some national account contracts. Contact your booking agent, transportation management office, or move coordinator for specific claim filing information.
C. Please have all shipping documents available at time of inspection, including the Descriptive Inventories.
D. Transportation charges must be paid in full prior to claim settlement.
E. The Order Number must be referenced on the claim form and any subsequent correspondence or inquiries. This number can be found at the top right corner of the Bill of Lading. This number also appears on the top right corner of the Order for Service.
 
F. Complete the 'Customer Information' section of the form thoroughly. Include zip codes with addresses and area codes with telephone numbers. Please give us the phone numbers where you can be reached during normal business hours.
G. Complete all fields for each article claimed:
  1. Not providing Inventory Numbers may delay the processing of your claim.
  2. Give a brief description of each article claimed including make and model number if applicable.
  3. Describe the nature, location, and extent of damage.
  4. If the claimed item is missing, enter 'missing'.
  5. Indicate the article's replacement cost for the same or like articles.
  6. Enter the amount you are claiming in settlement. The claim form is not complete without this amount.
  7. If the claimed item was packed, please choose whether the carton was damaged by selecting Yes or No in the appropriate field.

H. Be sure all unpacking has been completed and all items examined before submitting claim.

I. Do not have any items repaired unless your assigned claims adjuster advises you to do
so in writing.
